CAREGIVER’S SUPPORT NETWORK
1. SIGNING UP:
Parents must complete the following paperwork before they can use the sick care service.
After each form is completely filled out, the parent/guardian must return these forms to the Cabinet. After Cabinet Staff review the paperwork, parents will be notified regarding the status of their child(ren)’s enrollment. If the forms are incomplete, for example, the family will be notified of what is needed in order to complete the registration process. If all paperwork is completed correctly a copy of all completed paperwork will be sent back to the parents along with instructions on how to use the service. The family will then be eligible to use the service for those children who are registered.
Parents must complete the following paperwork before leaving the CSN Caregiver alone with the sick child. Paperwork must be completed for each day of service.
The parent or guardian needs to plan to spend a half-hour with the CSN Caregiver filling out these forms and answering questions before he or she can leave the child alone with the Caregiver.
Each child must be registered separately.
If an authorized person other than the parent is to assume responsibility for the child(ren) at end of service day, identification will be required.
