• Only one form per family, this helps us provide support to as many families as possible this holiday season. 
• Please inlcude your cell phone number. This is how we will contact you and keep track of your sign-up.
• After you submit the online form, this is nothing else you need to do. You will receive a message once your toys are ready for pickup. This will be toward the end of November or in early December. 
• By submitting this form, you confirm that you have not received holiday assistance from any of our partner organizations in the community.

Pickup Deadline: 
All toys must be picked up by Saturday, December 20th

Eligibility: 
All children receiving toys must be under 18 years old.
